Abstract / Summary
Abstract Left atrial and right ventricular deformation have been studied less than left ventricular global longitudinal strain during cancer therapy, and three-dimensional measurements of both chambers have not been reported in a mixed-tumour cohort. Forty-five adults starting immunotherapy (n = 26) or targeted therapy (n = 19) underwent echocardiography and electrocardiography at baseline, 3 and 6 months. Three-dimensional left atrial reservoir, conduit and booster strain and right ventricular global longitudinal and free-wall strain were measured offline. From baseline to 6 months, right ventricular global longitudinal strain fell from 21.4% to 18.2%, free-wall strain from 25.3 ± 3.8% to 21.7 ± 4.4%, and left atrial conduit strain from 20.0% to 15.0% (all p < 0.001). Reservoir strain fell modestly (p = 0.005) and booster strain did not change. No parameter changed between 3 and 6 months, and conventional right ventricular indices and chamber dimensions changed in parallel. New-onset arrhythmia occurred in 20 of 40 participants free of arrhythmia at baseline, most often sinus tachycardia, and no strain measure identified those in whom it occurred. In this uncontrolled cohort, right ventricular and left atrial conduit deformation declined within three months of therapy; without a comparison group these changes cannot be attributed to treatment.
