Abstract Introduction Bronchoscopic biopsy of multiple pulmonary lesions is typically performed sequentially, often requiring repeated imaging confirmation during cone-beam computed tomography (CBCT)–guided procedures. Case presentation We report a novel technique using simultaneous dual cryobiopsy with two bronchoscopes to sample separate lesions during a single CBCT acquisition. A patient with multiple lesions in the left upper lobe underwent bronchoscopy under general anaesthesia. Two bronchoscopes were inserted simultaneously through a 9 mm endotracheal tube: an Ambu aScope 5 Broncho (4.2 mm outer diameter, 2.2 mm working channel) with a 1.9 mm reusable cryoprobe and an Olympus MP190F bronchoscope with a 1.1 mm cryoprobe. A single CBCT spin confirmed simultaneous tool-in-lesion positioning for both cryoprobes. Cryobiopsies were obtained successfully from both lesions. Histopathological examination revealed squamous cell carcinoma. Conclusion This case demonstrates the technical feasibility of simultaneous dual cryobiopsy and suggests a potential strategy to improve procedural efficiency and reduce imaging requirements during CBCT-guided bronchoscopy.
