Abstract / Summary
Abstract Background Paraduodenal hernias are rare and the right side is less common than the left one. Recently, the right paraduodenal hernias have been classified into three distinct groups based on anatomy and physiopathology of the hernia mechanism. The surgical treatments described are sac closure, sac resection with hernia neck widening, and Ladd’s procedure according to the types. The sac closure technique is the preferred method for the type I PDH (paraduodenal hernia). No standardized treatment or clear recommendation exists concerning the techniques because of the rarity and heterogeneity of this pathology. Case presentation The case involves a 45-year-old Caucasian woman, who presented to the emergency with acute abdominal pain. A CT scan allowed to make the diagnosis of a right paraduodenal hernia, which was later identified as type I. She was managed by emergency surgery with a laparoscopic approach and closure of the sac in an inverted fashion without closure of the neck as shown in our video. The postoperative course was uneventful. No recurrence one year later. Conclusion Right paraduodenal hernias are rare and varied. Their treatments differ according to their subtype. The laparoscopic approach has been described and deemed safe with varying techniques. We present our variant of sac closure by plication concerning a type I PDH treated laparoscopically, with a video vignette.
