Hydrodissection technique for persistent macularHoles- Intraoperative OCT findings: Case Reports
By Anjian Wang, Takashi Koto and Makoto Inoue.
Abstract
Introduction
To present the intraoperative optical coherence tomographic (OCT) findings of the hydrodissection technique used for persistent macular holes (MHs). Case presentations: We present our findings in a 63-year-old man with vision of 20/50 and a 50-year-old woman with vision of 20/100 due to persistent MHs after vitrectomy and internal limiting membrane (ILM) peeling. During vitrectomy, balanced salt solution was injected into the MH, then the MH edge was gently massaged with a soft-tip silicone cannula, and it was positioned to close the MH. An ILM flap was created by the residual ILM near the vascular arcade and inserted into the MH. Intraoperative OCT showed that the size of intraretinal cysts at the edge of MH decreased, and the minimal MH diameter decreased to 91% and 76%, respectively, after the massage. The basal MH diameter increased to 113% and 103%, respectively. The MHs were closed by a gas tamponade, and the vision improved to 20/40 and 20/30 at 6 months, respectively.
