Abstract Brow lift surgery has evolved from traditional open to minimally invasive endoscopic approaches. However, consensus regarding their outcomes and complications remains lacking. This article aims to compare open and endoscopic brow lift by aesthetic outcomes, complication rates, success, and patient satisfaction. This is a PRISMA 2020 systematic review. Literature review of multiple databases from March 2005 to August 2025 was conducted. Studies reporting outcomes of open and/or endoscopic brow lift were included. Quantitative analyses were weighted by cohort size. Fifteen studies were analyzed. Endoscopic techniques demonstrated significantly lower complication rates compared with open approaches (p < 0.001). Open brow lifts achieved higher aesthetic scores at the midpupil and lateral canthus with slightly higher success rates and patient satisfaction. Overall satisfaction remained high for both techniques. Both open and endoscopic brow lift techniques are effective and safe. Endoscopic approach offers lower morbidity, while open technique provides greater aesthetic outcomes.
