Fluid Challenge in the Hemodynamic Gray Zone: Is It Time to Lower the Bar?
By Rachel Goodman and Vishal N. Rao.
Abstract
As the prevalence of heart failure with preserved ejection fraction (HFpEF) continues to rise, ensuring timely and accurate diagnosis is increasingly important. Yet, HFpEF remains a diagnostic challenge, particularly among patients with unexplained exertional dyspnea and normal resting hemodynamics. Its symptoms frequently overlap with obesity, chronic lung disease, and atrial arrhythmias.1 While evidence of congestion on physical exam and breathlessness may suggest HFpEF or left ventricular diastolic dysfunction (LVDD), up to 35% or patients may present with otherwise unexplained dyspnea and evade diagnosis by resting noninvasive or invasive testing alone.
