Double-Patch Sandwich Closure of Late Ventriculo-Atrial Communication After Mitral Valve Replacement
By Toru Tsukada, Ryuhei Yamamoto, Bryan James Mathis and 2 others.
Abstract
A 62-year-old man developed progressive heart failure and hemolytic anemia 11 years after mechanical mitral valve replacement (MVR). Echocardiography showed severe paravalvular regurgitation caused by a localized ventriculo-atrial communication near the anterior commissure. Intraoperatively, the prosthesis was stable and noninfected, but the annular tissue was friable. Redo valve replacement was avoided and the defect was closed with two autologous pericardial patches placed on the ventricular and atrial sides in a sandwich fashion. At 32 months, there was preserved prosthetic function, marked reverse remodeling, and no recurrent shunt.
