Abstract Radioactive Iodine-refractory Differentiated Thyroid Cancer (RAIR-DTC) has a poor prognosis, and the management of local recurrence or metastatic lesions remains a clinical challenge. 125I seed, as a form of brachytherapy, delivers a high dose of low-energy radiation to the tumor continuously. Due to its short radiation range and ability to protect surrounding normal tissues, this technique is suitable for lesions that are inoperable or have less response to external beam radiotherapy, such as cervical lymph nodes and bone metastases. Current studies have demonstrated that ¹²⁵I seed brachytherapy can effectively reduce RAIR-DTC tumor volume, alleviate local symptoms, and lower serum thyroglobulin (Tg) levels, with a favorable safety profile. Additionally, 125I seed have been use for preoperative lesion localization and assist in the precise resection of metastases in complex locations. This article reviews the clinical application value of 125I seed brachytherapy in the treatment of RAIR-DTC.
