Abstract / Summary
Abstract Purpose of Review This review examines recent innovations in organ delivery across three domains: organ preservation and ex vivo perfusion, digital and predictive transport infrastructure, and physical delivery vectors including advanced air mobility and unmanned aerial vehicles (UAVs), summarizing the current evidence and maturity differences across each domain. Recent Findings Extended ex vivo perfusion and normothermic regional perfusion have lengthened viable transport windows and improved graft function. Artificial intelligence and real-time telemetry tools for allocation and logistics show growing interest but remain early-stage. UAV-based delivery has progressed from isolated demonstrations to systematic feasibility analysis, though substantial regulatory barriers remain. Summary No single innovation outperforms the others; each addresses a different point along the transport chain. Preservation extends available time, digital tools inform its use, and physical transport determines delivery speed and reliability. Comparative-effectiveness research and infrastructure investment are needed before any approach is clearly preferred.
