Skin aging is driven by accumulation of senescent cells, chronic low-grade inflammation (inflammaging), and exhaustion of the dermal regenerative pool. The senescence-associated secretory phenotype (SASP) perpetuates matrix degradation and immune dysregulation.

To evaluate the clinical and biological efficacy of a sequential regenerative protocol – neofibrolifting – in modifying the senescent tissue microenvironment and reversing involutional-dystrophic skin changes.
A prospective study included 107 women aged 25–60 years, divided into four age groups. The protocol consisted of: (1) three intradermal injections of autologous platelet cryolysate (every 7 days); (2) transplantation of cultured autologous dermal fibroblasts (≥60 million cells); and (3) ablative fractional photothermolysis after 6 months. Outcomes were assessed by corneometry, ultrasound dermal thickness, volumetric blood flow velocity (Doppler), and skin immune cell profiling (CD4+, CD8+, CD19+).
Dermal thickness increased by 11–28% at 12 months (p<0.05). Hydration improved by 26–37% (p<0.001). Volumetric blood flow increased up to 4-fold in the oldest group (56+ years), sustained at 12 months. Immunologically, CD4+ T-helper cells increased 1.4–2.5-fold, CD8+ cytotoxic T-cells decreased, and CD19+ B-cells decreased by 18–39%, normalizing the CD4+/CD8+ ratio toward young skin values.
Neofibrolifting improves dermal thickness, hydration, microcirculation, and cutaneous immune homeostasis for at least 12 months. The approach targets the senescent microenvironment and represents a true biological rejuvenation strategy beyond cosmetic correction.
