Résumé
FORMHANSEN is a standardized neurodermatological assessment form designed to support the clinical detection and documentation of Hansen’s disease in contacts, walk-in patients, active case-finding activities, and field campaigns. It integrates the recording of complaints, facial assessment, peripheral nerve palpation, muscle strength, sensory testing with monofilaments, physical disability grading, skin examination, anatomical mapping of lesions and sensory changes, and the clinical conclusion. The accompanying completion guide provides detailed instructions for each section, including the criteria for counting affected nerves. Pain or electric-shock sensation on nerve palpation, when present in isolation, must not be used to establish a diagnosis of Hansen’s disease. However, once the diagnosis has been established, every nerve in which pain or electric-shock sensation was recorded must be counted as affected. The record contains the FORMHANSEN form and its completion guide in Portuguese and English. The instrument was developed by the Dermato-Immunology Laboratory of the Federal University of Pará (LDI/UFPA), drawing technical inspiration from the Simplified Neurological Assessment and using anatomical references adapted from Gray’s Anatomy for Students and Evidence-Based Medicine Consult. FORMHANSEN is free to use, provided that the source and DOI are properly cited. It does not replace clinical training, professional judgment, applicable clinical protocols, or manufacturers’ instructions.
