Abstract
Medication reconciliation is a crucial process for patient safety, particularly during admission to emergency care units. This study aimed to describe and quantify medication discrepancies identified during medication reconciliation among patients admitted to an Emergency Care Unit in Sinop, Mato Grosso, Brazil. We conducted a prospective observational study with a quantitative design involving a convenience sample of 60 patients admitted to the unit between 31 January and 10 April 2025. Medication discrepancies were identified in 41.7% of patients. Of the 58 discrepancies identified, 40% (n = 23) were intentional and 60% (n = 35) were unintentional. Medication omission was the most common type of unintentional discrepancy, accounting for 66% (n = 23) of cases. These findings highlight the importance of medication reconciliation in minimizing errors and ensuring safe and continuous medication therapy in emergency care settings.
