Abstract
Influenza vaccination remains a key component of paediatric prevention during the 2026-2027 season. Italian recommendations identify children aged 6 months to 6 years among the main target groups for vaccination, together with individuals of any age at increased risk of influenza complications, while the American Academy of Pediatrics recommends annual vaccination for all children aged 6 months and older. Particular attention should be paid to children younger than 9 years who have not been adequately vaccinated in previous seasons and therefore require two doses at least four weeks apart. Vaccination should be offered as soon as it becomes available, especially when two doses are needed, because vaccination may still provide benefit later in the season while influenza viruses are circulating. Available options include injectable inactivated vaccines from 6 months of age and the live attenuated intranasal vaccine from 2 years, when clinically appropriate and available. Vaccination reduces the risk of influenza and its complications but does not completely prevent infection. In children with severe or progressive disease, those who are hospitalised, or those at increased risk of complications, early antiviral treatment should therefore be considered. This article summarises the main practical points for paediatric influenza prevention and management in the 2026-2027 season.
